Our Patient Questionnaires are optional and provided for your convenience. Download, print, fill out and fax the Questionnaire(s) applicable to your appointment to (407) 740-0324. This will save you time. If you can't fax it, please bring it with you.

Patient Preparation:

• Avoid drinking carbonated beverages three hours prior to a lumbar spine or abdomen exam
• Avoid caffeine the day of your appointment
• Bring your insurance I.D. card and any previous x-rays
• Wear warm, comfortable, loose clothing without metal preferred
• Brain scan patients should NOT wear eye makeup
• Bring your favorite music CD
